Due to COVID-19, we are asking all patients to please download and sign supplemental Health Questionnaire and Informed Consent forms. These forms can either be printed, signed, and brought to your appointment or emailed to terry@drdaveortho.com. Informed Consent only needs to be signed once, but a Health Questionnaire MUST be signed for each visit. Please feel free to contact us if you have any questions. Thanks!
